After a severe medical condition or event, many people are unable to communicate their wishes. Unfortunately, these significant health events often result in difficult decisions being made by loved ones. By completing an Advance Health Directive, you can make your wishes known before this happens.

An Advance Health Directive is a document that states your wishes or directions regarding your future health care for various medical conditions. Importantly, your Advance Health Directive can only be used when you cannot make your own healthcare decisions.
